Summary of missions:
The assessor carries out his duties under the responsibility of the coordinating nurse, who is responsible for the Nursing Home Service for the Elderly.
Its mission is:
- to promote the existence of an acceptable life project for each beneficiary of the personalized home allowance (APA);
- establish the collection of personal data of the elderly person or family, APA applicant;
- analyze the data collected to determine the needs of the person and prepare an assistance plan based on these needs;
- to prepare the invoices and the balance sheet of the files by informing the General Council;
- to have a close relationship with the various partners (doctors, General Council etc.)
Activities:
The assessor assesses at home the needs of the dependent elderly.
Technical activities:
- Scheduling appointments with the secretary
- Home evaluation
- Using the AGGIR grid
- study of the environment: home improvement, technical aids
- Prediction of the adapted equipment (medical bed, wheelchair, anti-slip mat for safety)
- Establishment of a caring relationship between the elderly, professionals involved in the home (auxiliaries, SSIDPA, nurses, pedicure, doctor, social worker)
- Solicitation of the participation of the carer and his entourage
- Regular needs assessment and readjustment of needs regularly Quality audit of interventions regularly
- Competition for the prevention and education of elderly people evaluated (increased monitoring in case of heat wave)

Required profile
Knowledge required:
The evaluator contributes to the care of the A.D.P.A. elderly dependent or ill.
He coordinates the activities of the various workers in his home to meet the needs of maintenance and continuity of life and to partially or totally compensate for a lack or a decrease in autonomy of the person supported by home care .
It must ensure a follow-up of the elderly in partnership with the treating physicians, the liberal nurses and the family, the General Council, etc.

Required skills:
- Hold a state diploma as a nurse or counselor in social and family economics
